Why I Created These Forms
Caring for my father taught me that the right piece of paper, at the right moment, can be the difference between panic and calm.
At first, a notebook and a few sticky notes were enough. Then medications multiplied, appointments overlapped, and the simple systems I had originally created were no longer enough. I found myself scrambling in emergency rooms and pharmacy lines, trying to remember dosages, allergies, and doctor names while someone I loved waited beside me.
I started designing these forms because I needed something I could hand to a nurse, a specialist, or a family member without explaining twice. One page for medications. One page for emergencies. One page for appointments. Simple, clear, and ready when life isn't.
